Diabetes Registry Update
 
 

Disease Registries are collections of data regarding specific groups of patients with similar chronic conditions, such as diabetes, or congestive heart failure. The Registry helps us keep track of those with chronic illness, and see how we are doing at "managing" these illnesses, all with the goal of facilitating and improving their care. 

Our Diabetes Registry incorporates all patient of the practice with know Diabetes. In 2007, when we started tracking these patients, only 40% exhibited a Glycohemoglobin (a measure of the average blood sugar over the preceding two to three months) at target (less than 7 for most patients). Currently 64% are at target.  We congratulate our patients!  Diabetes Registries have been shown to improve the care of populations,  as we are seeing this in our small practice. Our Diabetes Registry also allows us to identify those who are having more difficulty controlling their diabetes, so we can work more diligently with these patients. 

By completing those "enrollment" packets, you were (perhaps unwittingly) helping us improve your health, as well as the health of your fellow patients. This allows us to identify medical conditions of patients of the practice, and thereby allocate our energies were they are needed most.  THANK YOU!
