Enrollment
We currently have a waiting list of those wishing to enroll in our practice. Should you wish to be included on this list, please follow the directions below to send us an e-mail. Please include your name, a contact phone number, and number of family members to be included. Also, please let us know how you came to learn about our practice. We will email, or call you as spaces open and forward an enrollment packet for you to complete. Once we have received a completed packet, we are ready to assume your care.
We hope our practice fits your needs and want to make enrollment as easy as possible.

We are glad to discuss our practice and your needs by phone, or in person. We welcome all questions. We’d also be happy to arrange a “get a look at us” visit with Dr. Bloomer or Dr. Worrell. No fee, no obligation. We want to be sure we are just what you need. Simply call or e- mail the office to ask a question or arrange a time to meet.
